Information used to administer a participating account includes organization and facility names, service addresses, authorized representative details, billing and service contacts, email addresses, telephone numbers, signatures, and installation details.

Our program records also include equipment assignments, token quantities supplied and remaining, service communications, repair reports, invoices, payment status, and payment authorizations where applicable.

 

How Information Is Used
We use this information to evaluate participation requests, coordinate installation and regional servicing, supply tokens and prizes, respond to equipment issues, administer billing, communicate with authorized contacts, and document our agreements and service obligations.

 

Repair Photos and Sensitive Information
A repair report may include photographs or a short video of the equipment. Please focus these materials on the equipment and avoid including children, patients, students, medical information, student records, or other unrelated personal information.
